How do I write therapy session notes?

Session notes are short, factual and written right after the session. Below you see what goes into a note, what data protection requires and how to keep them in the client record.

Short answer

  1. Write the note right after the session, while the details are fresh.
  2. Record only what is necessary: the session theme, the interventions used, progress and the next step.
  3. Use a fixed format so you can compare sessions with each other.
  4. The note stays in the client record, linked to the session appointment.

What goes into a session note

The structure of a session note

ElementWhat you record
Session themeThe main topic discussed, in one sentence
InterventionsThe techniques or exercises used in the session
ProgressWhat changed since the previous session
Next stepHomework or the topic of the next session

What data protection requires (in brief)

  • Mental-health data are sensitive data and require a clear legal basis for processing (GDPR, art. 9).
  • Record only what is adequate and necessary for the therapeutic purpose (data minimisation, GDPR, art. 5(1)(c)).
  • The client can request access to their data (GDPR, art. 15), so write every note as if it could be read.
  • Professional secrecy also covers the notes: access should be limited to authorized people.

How to write them, step by step

  1. Open the client record right after the session.
  2. Fill in the four elements: theme, interventions, progress, next step.
  3. Keep the language factual, without interpretations you cannot support.
  4. Check that the note is linked to the right appointment and move on to the next session.

Practical example

Example: after a 50-minute session, the therapist writes four lines — the theme (sleep and evening anxiety), the intervention (a breathing exercise), progress (two calmer nights compared with last week) and the next step (a sleep diary). The note is saved in the client record, on Tuesday's appointment.

Common mistakes

  • Writing notes days later, from memory, and losing important details.
  • Recording too much, including data that does not serve the therapeutic purpose.
  • Using a different format every session, so progress can no longer be followed.

How 4therapy helps

  • Session notes in plain text, added to the client record after each session.
  • A single electronic record per client, with history and session progress in one place.
  • The appointment is linked to the record, so the history is visible before every session.
